Teacher Observation Form for College Advisor
Please note: this form is NOT sent to colleges. The purpose of this instrument is to provide the college advisor with significant information in creating an overall school recommendation.
The information is the most helpful when it cites specific examples of the student's work ethic, interest in your subject, examples of his going above and beyond what is required.

How has this student performed in your classroom? For example, cite his inquisitiveness, hard work, participation, and contribution to discussion and evidence of research skills. How did he respond to short and long range assignments? Did he work to his potential? Most important: is there an anecdote you can recall from your classroom time with him?
What do you know of this student outside the classroom? What examples can you cite of his participation in school sport or club activities. What can you write about his leadership ability in these activities? What examples of his being a positive role model can you cite? Can you cite his community activities off-campus? If you are not familiar with these, just leave them blank.
What strengths will he offer a college?
How does he respond to setbacks? Correction? Defeat?
